Aloha.

Persuant to UIPA, I request to see electronic copies of all documents describing how amended and delayed birth records in paper form are converted to electronic form and into a printed document when requests are made for the birth record – including, but not limited to, documents describing when and how a void flag is to be entered into the electronic record and what output is generated on a COLB or other printed form to identify that the record is void. I also request to see any documents which describe the impact that a void flag has when a verification is performed on that record in the national EVVE System, in which Hawaii participates.

I look forward to receiving these records by the end of the work day on Dec 16th.

We do not have a record responsive to your request. The UIPA does not require an agency to compile and create information to respond to a request.
