MCFD Incident Report

MCFD Incident Report Contact Info Deleted

This is the PDF I was sent by the Maui County Fire Department regarding the Dec 11, 2013 Cessna crash off Kalaupapa, minus the request form which had my contact information.

As you read this there’s something you should know: this has been chopped up and is out of order. The narration actually begins on page 7, with this, and ending with what is at the end of the …:

CA20131211153730HS**Case number FC 130015086 hs been assigned for MFD-F5……….  CG WANTS 10-2 WHEN AIR1 IS HEADI

Everything in that section is supposed to be at the start of the CAD narrative, which as it stands now starts on p. 5 with

NG OUT (?) 20131211155151 HS ADVISED THE BC IS MAKING THAT DETERMINATION

What this misplacement hides is that the CAD narrative ends this way:

20131211171942 HS KPAA CMMD…USCG HAS 1 CTITIAL TRYING TO FIGURE OUT WHERE TO FLY IT OAHU OR MAUI 20131211172128 HS USCG – 7 PAXX TOTAL ON SHORE…USCG HAS 1 BOARD CRITAIL.

IOW in 2013-12-11, at 17:19:42 (or 5:19:42pm) there were 7 people on shore, with one of them being on board USCG helicopter in critical condition, and they were trying to figure out whether to fly them to Oahu or to Maui.  At that point MDFC’s AIR1 and Maui Medevac helicopter were both still there at Kalaupapa, as were the 2 USCG choppers. Incident commnd on topside Molokai needed to keep Maui Memorial Hospital notified if patients were coming to them. Yet the CAD narrative ends there with a cliff-hanger.

The written narrative – which was written the Sunday after the crash (which was on a Wednesday), was “reported” on Dec 29th, and “reviewed” on Jan 22 (5 days AFTER I requested all records related to this incident, not just the incident report) – says that of those 7 on shore, 2 victims declined transport, the USCG transported 3 to Honolulu, MCFD’s AIR1 transported 2 in stable condition to the staging area on topside Molokai, Maui Medevac transported 1 to the topside staging area, and Maui Medevac transported one dead body to the topside airport. IOW, of the 7 people on shore, the status was reported for NINE.

If the person in critical condition was Fuddy and she died, then when the media reported one fatality at 4:40, that was roughly an HOUR before anybody WAS dead – and the stories of Fuddy dying in the water and being found dead by the rescue swimmers are all false.

If the person in critical condition WASN’T Fuddy then there was at least one extra person in the reports – including one who was in the WORST shape of anybody, who all the official reports happened to leave out.

The numbers were always a mess.

At one point it was said that 2 had swum ashore, another time it was said that 1 had swum ashore and 1 person was still missing in the water,  another place it said a “few” had made it to shore but there were still “several” in the water… And there is one place where it is even acknowledged that the counts were probably being confused because of extra people – people reported as having swum ashore but NOT part of the 9 who were supposed to have been on board:

2013121117042HS US PO PETTY CONFIRMED 3 PICKED OUT THE WATER…5 BY AIR1…NOT SURE ON SHORE WAS PART THE AIRCRAFT

This was right after someone referenced ELEVEN victims:

20131211170620HS PIO JUAN UPDATED – AIR1 PICKED UP 5, CG PICKED UP 5, 1 UNACCOUNTED FOR.

So the extras being seen in the water in Puentes’ video also seem to be showing up in this cut-up incident report.
